Update on amended burial and cremation forms

In our blog post on 4 February 2026, we informed everyone about the new burial and cremation application forms that are coming into force on 2 March 2026.
Our intention was to share the new forms in advance to allow burial and cremation authorities time to add their logo and address. We apologise that unfortunately we have experienced delays in production however we anticipate receipt of these tomorrow.
You should continue to use the existing burial and cremation application forms until we are in a position to circulate the updated versions. This includes applications you receive the week commencing 2 March 2026. The link for the current forms are below:
We will share the PDFs as soon as we have the final versions.
